Active Eyecare of Surprise is located in the Surprise Professional Office Park on the north side of Bell Road, between Cotton Lane and the Loop 303.  You can enter from Eastham Parkway or the access road next to the new Midfirst Bank.  We are located in the fourth building down from Eastham Parkway, behind Canyon Physical Therapy.
